Sildenafil was originally known as compound UK-92,489 and was originally manufactured to treat angina pectoris otherwise known as a form of cardiovascular disease. Phase I: clinical trails proved that the drug ended up having very little effect on angina but was discovered to stimulate obvious penile erections. Pfizer then decided to promote it for erectile dysfunction instead of angina as initially planned. This pill was then patented in 1996 and approved to help in the aid of erectile dysfunction by the FDA on March 27, 1998, thus becoming the first ever approved medication to treat erectile dysfunction in the United States and presented for sale in the United States in 1998. This medication was soon to be a great success: yearly sales of Viagra surpassed $1 billion in the period in between 1999 - 2001.
The names of drugs like "Viagra" are creatively selected to market the product. The name "Viagra" could have been brilliantly chosen since it could relate to the Sanskrit word "viaghra" that also means "tiger". Viagra also rhymes with the famous honeymoon destination other wise known as Niagara Falls. (Niagara Falls has remarkable force, induces a sense of astonishing and ejaculatory strength). The words "vigor" and "virile" also sounds like and can relate to the word "Viagra".
Viagra is currently a prescription medication even though it has been advertised on TV (being promoted by the extremely well known Bob Dole) to consumers like you. You can such for sites on the Internet that advertise the availability of Viagra after an "online consultation" which consist of filling out a web questionnaire. You often have to keep an eye out for fake products since the name "Viagra" has became so popular other products not associated with "Viagra" comes about. Such items include fake aphrodisiacs that are known as "herbal Viagra" or pills that have the same shape and color of the genuine product. A Viagra is unofficially known as "Vitamin V" among other nicknames. Partygoers also has became familiar with a drug they know as sextasy it is a mixture of Viagra and ecstasy.
Some suggest that changing the name of Viagra to something like "tiger penises" or "rhinoceros horns" would have a decline in market sells as well as preserve these endangered species. In these parts of endangered species are not used to treat impotence. For example, to treat a high fever the use of rhinoceros horns are likely to be used. In view of the fact that Viagra has not been proven to possess aphrodisiac properties it is unlikely that a natural remedy would compete with this clinical proven drug.
Between 2011 - 2013 the Pfizer's worldwide patens on sildenafil citrate will than expire. The PDE5 inhibitors known for treating impotence has a patent held by Pfizer (see below) which been invalid around the time of 2000 since the openness, the decision was than upheld on appeal in 2002.
The process of the erection is in part physiological which implicates the release of nitric oxide (NO) in the corpus cavernosum. This then sets in motion the enzyme guanylate cyclase that then effects the increase levels of the cyclic guanosine monophosphate (cGMP), thus resulting in an increase the incoming blood and then the final result is the erection.
Sildenafil is a powerful and perceptive inhibitor of cGMP much like phosphodiesterase type 5 (PDE5) that has been accountable for the ruin of cGMP in the corpus cavernosum. Which means that, with a drug like Viagra on board, usual sexual motivation leads to amplified levels of cGMP in the corpus cavernosum that leads to an enhanced erection. With no sexual motivation and no commencement of the NO/cGMP system, thus an erection should not be caused with the use of sildenafil. A few medications that work similarly include tadalafil (Cialis®) and vardenafil (Levitra®).
Sildenafil is sped up by hepatic enzymes and released by two major organs such as the liver and kidneys. A meal high in fat may delay the absorption of Viagra the ultimate results would be affected by slightly as the concentration of plasma would be decreased.
Recent studies have declared that sildenafil has caused an improved sexual pleasure for women simply by escalating the amount of blood flow to the sexual organs.
Only a licensed medical physician has the judgment to properly prescribe and distribute this drug in the accurate milligrams and pill amounts. Sildenafil can be issued in 25mg up to 100mg tablets which should be taken once a day approximately .5 to 4 hours before sexual interaction.
The first recommended dosage should start out at 50 mg or lower then raising the dosage as appropriate. This medication is sold in 25, 50 and 100 mg tablets, all of which cost approximately $10 per pill (USD). With Sildenafil there is no guarantee that this medication will evenly disperse throughout the tablet therefore it would not be advised to cut down or change the dosage.
Studies show that men are at an increased risk and should be advised when taking other nitric oxide contributors, organic nitrites and nitrates such as glyceryl trinitrate, sodium nitroprusside, amyl nitrite (Cheitlin et al 1999). These drugs are not recommended if a man is advised that sexual intercourse is not recommended due to cardiovascular threat, severe hepatic impairment (decline in liver functions), sever impairment in renal function Hypotension (low blood pressure), stroke or heart attack, inherited degenerative retinal illness (including inherited disorders of retinal phosphodiesterases). Sildenfil's most serious adverse effects are as follows: priapism, severe hypotension, myocardial infarction, ventricular arrhythmias, sudden death, stroke and increased intraocular pressure.
Studies have shown more common side effects such as sneezing, headache, flushing, dyspepsia, prolonged erections, palpitations and photophobia. Be aware of some visual changes that have been reported such as blurring of vision and a curious bluish tinge.
In May of 2005, the U.S. Food and Drug Administration establish that sildenafil was proven to cause vision impairment. This finding is currently under investigation. After taking sildenafil some patients developed nonarteritic ischemic optic neuropathy (NAION), which is an eye problem that can develop into permanent loss of vision. Recent reports show the number of cases of NAION that was report to date was 14.
Some patients that use sildenafil has complained of blurriness as well as some patients who lost their peripheral vision. In all reported cases the patient had a hereditary condition known as a "cup" in the retina.
Studies have also proven that aside from treating erectile dysfunction, sildenafil citrate is also very successful in the rare disease known as pulmonary arterial hypertension (PAH). The drug seems to relax the arterial wall, lessening the pressure on the lung vasculature while improving symptoms of right-sided heart failure. As the effect of the findings in these studies, Pfizer presented an additional registration for sildenafil with the United States Food and Drug Administration which was approved for this suggestion in June 2005. It would seem most likely to avoid confusion with Viagra, the research was prepared under and named Revatio and the 20 milligram tablets are white and round. Sildenafil pursue bosentan and prostacyclin as treatment for this condition.
